A parent/guardian and their new high school student(s) will need to be present on the
day of registration.
Please bring the required 4-5 documents:
(1) birth certificate,
(2) social security card,
(3) paper copy of immunization record (must be pre-cleared by LCMHS nurse*),
(4) proof of residency (electric, gas, water, sewer, or lease),
(5) the latest transcript or report card if student has been enrolled in another district.
*Immunization Record - Texas requires that EACH student's immunization record
must be up to date and cleared through our school nurse prior to registration
whether coming from another Texas school or from out of state. At least 48 hours prior
to your registration day, please email the student’s immunization record to the LCM
high school nurse Jeannie Alexander at jalexander@lcmcisd.org. She will review the
records and confirm by email if any shots are needed before registration.
